We are contracted with all major health insurance carriers. We will gladly bill your health insurance for you at the time of your visit in our office. Please present your insurance card at the time of your visit in order for us to bill your particular company. If you do not see your insurance company listed here, please contact our office at 624-6500 and we will be happy to help assist you in finding out if your plan is covered at our facility. Additionally, to those covered by state insurance plans, we do expect you to kindly bring a copy of your card to each visit.
